Collaborations

TFC uses a collaborative approach to all our work. Our commitment to working collaboratively within our community to meet unmet healthcare needs means that we find common cause with a wide variety of local organizations.

The Community Case Management program exists because providers in Henderson County understand that individual patients and our community as a whole is better served when all participate in a system of sharing the provision of care for the medically indigent. There are currently 84 providers who each agree to see patients who need access to care, understanding that the patients have met TFC's eligibility requirements and are receiving case management support through TFC.

Primary collaborative partners include Pardee and Park Ridge Hospitals, Blue Ridge Community Health Services, Hendersonville Family Health Center—Hendersonville and Etowah offices, Flat Rock Family Health Center, Henderson County Department of Social Services, Access II Care of Western North Carolina, NC Association of Free Clinics, Sixth Avenue Rehabilitation Partners, and Partnership for Health.

Department of Social Services

Thanks to a collaborative partnership with the Henderson County Department of Social Services, we host an Adult Medicaid Worker on-site at our facility.  This relationship eases patient access to services--both for those who qualify for Medicaid as well as those who do not, but for whom a "Medicaid denial" is required for additional services.

Partnership for Health:  Mental Health Roundtable

As an active member of the Henderson County Partnership for Health (Healthy Carolinians project), TFC hosts the monthly meeting of the Mental Health Roundtable.  Mental health providers serving Henderson County gather monthly with the local management entity (Western Highlands Network) to discuss gaps in care and share resources.
