Diabetes Life Management Clinic

Intensive program for newly diagnosed patients or for those patients who have diabetes that is not well-managed

Educational workshops on living with diabetes, diabetes support group, nutritional counseling, a personalized wellness plan, access to medication and supplies, clinical visits with a physician

We offer an intensive Diabetic Life Management Program for newly diagnosed patients or those patients who have diabetes that is not well-managed.  The program includes several components:  education, access to medications and supplies, appointments with practitioners for careful monitoring of diabetes, one-on-one appointments with nutritionist, and personalized wellness plan.

 

The program requires referral from a primary care physician.

All diabetes clinical visits and education meetings take place on Monday afternoon/evenings at 841 Case Street.

The Diabetes Life Management Clinic is currently staffed by a volunteer team consisting of three physicians, seven nurses, one registered dietician, one physical therapist, and 12 lay volunteers.  In 2010-11, the clinic provided 291 clinical visits.
