Office Hours:

Monday, Wednesday, Thursday, Friday
9:00am – 5:00pm

Tuesday
11:30am – 7:30pm

Polk office: M-F 8:00am-4:00pm

Administrative, clinical, case management, and prescription assistance staff are available to assist patients during regular business hours.

Clinic Hours

Medical Clinic Hours:

Tuesdays at 5:00pm.
Accepts walk-in patients.

Psychiatric Clinic:

1st & 3rd Tuesdays at 5:00pm. Accepts walk-in patients and appointments.

Other clinical services are by appointment only.

Pharmacy Hours

Pharmacy:

Monday 12:00pm -5:00pm
Tuesday 4:30pm -7:30pm
Wednesday 9:00am-1:00pm
Friday 9:00am -3:00pm
